The most recent release of the MP3 library included and update of SdFatLib to sdfatlib20121219. Previous to this, it included sdfatlib20120719. http://code.google.com/p/sdfatlib/downloads/list . It is a long shot, but perhaps reverting back, may provide more stable operation.
Where as it sounds like some connector issue on the SdCard. perhaps cleaning it out with alcohol. or firmly pressing on it from boot through use to see if it is a cold solder joint. or even Re-flowing the board. Which may not be a possibility.
